By filling out this form you agree to the terms and rules of play, children must be from ages 8 to 18, must be accompanied by an adult at all times and be prepared to get to take the stage when called by the manager.

All kiddos play at their own risk so please be aware that you are responsible for all aspects of your child's participation. Drum kit and keyboard will be provided. Your child is actively giving back to their community through participation so thank you for bringing a donation of Peanut Butter, Jelly or Fluff for the PB&J Jam. Parent or guardian agrees to promotional use of all video and pictures taken on stage by the organizers of the PB&J Jam and Crossborder Tourism & Recreation Alliance
